The YMCA of Greater Charlotte and Carolinas Medical Center
are partnering to provide you with a team of experts to help improve your total health. the Lifestyle Management & Medical Referral Program includes wieght management, fitness programming and medical guidance to give you a comprehensive approach to building a healthier lifestyle.
This program is for you if any of the following apply:
- Have never exercised before or have difficulty maintaining exercise
- Are overweight or just looking to lose a little extra weight
- Are looking to live a healthier lifestyle through balanced eating habits & physical activity
Medical guidance will be provided to you by our Registered Nurse for any of the following:
- Have a greater risk of developing chronic diseases including hypertension, diabetes, heart disease and some forms of cancer
- Have recently been diagnosed with a medical condition that would benefit from a healthier lifestyle
- Have recently completed a cardiac or physical rehabilitation program
Program Highlights
- Working with a team of experts in the areas of nursing, fitness & nutrition
- Up to 16-weeks of education & support
- Initial assessment & consultation with the Registered Nurse
- Glucose and cholesterol screening
- Nutrition assessment
- Weekly nutrition education, goal setting and accountability
- One-on-one guidance to set up a safe and effective fitness program
- Health Rish Appraisal - providing you a snapshot of your total health and areas for improvement
- Access to web based wellness tools
