Program Highlights
Working with a team of experts in the areas of nursing, fitness and nutrition Up to 16 weeks of education and support. Each session is eight weeks in duration Initial assessment & consultation with Registered Nurse, Dianna Burhans Cholesterol and glucose screening Health Risk Appraisal - providing you a snapshot of your total health and areas for improvement Nutrition Assessment Weekly nutrition education, goal setting and accountability Personal goal setting One-on-one guidance to set up a safe and effective fitness program Access to web based wellness tools

This program fee reflects one eight week session. You will have an option to enroll for an additional eight weeks which will include a post-program cholesterol and glucose screening for the same low program cost. The second eight weeks will feature additional lifestyle management education and tools for continued success. |

Medical guidance will be provided to you by the Registered Nurse for any of the following:
Have a greater risk of developing chronic diseases including hypertension, diabetes, heart disease and some forms of cancer Have recently been diagnosed with a medical condition that would benefit from a healthier lifestyle Have recently completed a cardiac or physical rehabilitation program
