Weight Management - Exercise - Medical Guidance
Regular exercise and healthy eating habits are important to your health. These habits are essential in the prevention and treatment of many different chronic diseases, as well as for overall health and lifestyle management. In addition to reducing your risk for disease, exercise also assists in maintaining a healthy body weight, reduces stress levels and
increases your energy level.
The YMCA of Greater Charlotte and Carolinas Medical Center are partnering to provide you with a team of experts to help improve your total health. The Lifestyle Management & Medical Referral Program includes weight management, fitness programming and medical guidance to give you a comprehensive approach to building a healthier lifestyle.
PROGRAM HIGHLIGHTS
• Working with a team of experts in the areas of nursing, fitness and nutrition
• Up to 16 weeks of education & support. Each session is eight weeks in duration
• Initial assessment & consultation with the Registered Nurse
• Cholesterol and glucose screening
• Health Risk Appraisal – providing you a snapshot of your total health and areas for improvement
• Nutrition Assessment
• Weekly nutrition education, goal setting and accountability
• Personal goal setting
• One-on-one guidance to set up a safe and effective fitness program
• Access to web based wellness tools
PROGRAM FEE
Members: $75
Non-Members: $100
The program fee reflects one 8-week session. You will have an option to enroll for an additional eight weeks which will include a post-program cholesterol and glucose screening for the same low program cost. This second eight weeks will feature additional lifestyle management education and tools for continued success.

This program is the right fit for you if you:
• Are looking to live a healthier lifestyle through balanced eating habits & physical activity
• Have never exercised before or have difficulty maintaining exercise
• Are overweight or just looking to lose a few extra pounds
Medical guidance will be provided to you by the Registered Nurse for any of the following:
• Have a greater risk of developing chronic diseases including hypertension, diabetes, heart disease and some forms of cancer
• Have recently been diagnosed with a medical condition that would benefit from a healthier lifestyle
• Have recently completed a cardiac or physical rehabilitation program
