The Medical Referral Program is designed to provide you with resources, guidance and encouragement to attain a healthier lifestyle. This six-week program is ideal for first-time exercisers, those needing assistance with making healthy choices, post acute rehab clients who want to continue an active lifestyle, or those with chronic or recently diagnosed medical conditions.  

The program includes weekly meetings, personalized exercise prescription and nutrition education. Healthcare provider referral required for entry. Referral forms are available in the Health Center. Financial assistance is available.
